In this piece of Fate side material Iskandar tries to get Waver and Taiga to hook up, a union which would surely have created an offspring of ungodly strength.
Taiga + Waver = OP
Their child would be a master swordsman on par with Saber in terms of skill and with an at least half-decent set of Magic Circuits to boast.

The reason Lancer called Hiyori "senpai" is because Heroic Spirits don't have the same sense of time as normal humans. Although a Heroic Spirit may have died at age 70, or 60, they will appear in their most suitable form while retaining all memories of their life.
The thing is, since Lancer died young, he doesn't have any memories of being old, so he basically never even got the chance to grow up. As such, his personality is stuck on that level that isn't really what you'd imagine when you summon a "Knight of the Lance." He's a boy stuck in the body of a teenager for all eternity.
That said, a Heroic Spirit originates from the Throne of Heroes; a metaphysical location for the souls of heroes, outside time and space. Because of that, every time a Servant is summoned, they are just a copy of the real Heroic Spirit; the real soul of a hero. And when they die, their memories don't return to the Throne save for special cases like Saber in F/SN. So Lancer isn't an ancient being, just a copy of an ancient being that can only recall its life and whatever the Grail taught it upon summoning, like how to overcome language barriers.
