Well there was one time that she was killed when she was using Excalibur, but that was an exception.

Well the one exception was when her attack was erased by Fragarch's ability.
It is possible, but the charge up time is small enough that it would be too risky to engage her when she uses Excalibur. I also doubt she would use the full charge of Excalibur as a close range attack.

In other words, from what I read from the wiki, you will die if she uses Excalibur since it takes less then seconds for the attack to come out. You can however fight her up close like how Sasaki, so she doesn't have a chance to channel it.

If she doesn't notice, and the distance is short then yes. The blast isn't going to somehow turn around and hit whoever's behind. Scratch that she has instinct, so she'll probably know.
