When you say form, do you mean what I'm looking for in a character profile?

Yep. Most RPs have a form stating what the information necessary is, example:

• Name
  
• Age
  
• Appearance
  
• Personality
  
• Powers
  
• etc.
  

It really depends on what the Rp is though.
