The format on how to make a new character is simple. It goes something like this:

Name: Toshiro Hitsugaya
Gender: Male
Age: 16 in human years
Series and author (required if crossover character, skip if oc): Bleach by Tite Kubo
Appearance and Avatar (worded or a picture, avatar can be completely different from the normal look, even gender):
Hitsugaya's normal appearance is a short young lad with white hair and blue eyes.

His avatar resembles him as a taller, more mature and older version of himself

Unique sword skill (must be unique to each character): Hyorinmaru: Sword gains the ability to manipulate ice for a short amount of time (30-60 seconds to be exact.)

Backstory (simple or detailed summary of why the character joined SAO):

Spoiler: Toshiro backstory.

The youngest of 13 Captains, Toshiro has had a lot of stress and responsibility put on his shoulders. Despite his serious facade, Toshiro wishes to take his stress out doing what he loves most. He then proceded to get SAO in his human form to relieve his stress.

Bored of his job, Hitsugaya picked up SAO to slash his stress away

I forgot to mention, if your character isn't a swordsman, don't worry, they can still learn, just at a slower pace than the already swordsmen. If they have existing abilities, make them compatible for a sword and if they don't, get creative and make it their style.
ex: Brawlers, fighters, stand users, etc.
ex: Someone like Bruno Bucciarati (Jojo's Bizarre Adventure: Golden Wind) will learn to use a sword and learn the mechanics at a slower pace than someone like Gintoki Sakata (Gintama). and Bruno's Zipper ability will carry over to his sword skill.

Backstory: The Son of Hades and Hero of Olympus, Nico lost so much at such a young age and gained just as much. While taking a break after the war he decided to borrow a nerve gear from the Hermes cabin and decided to try out the game called SAO while being from the 40s he had only a basic idea of how to play an RPG, he did have prior experience playing Legend of Zelda.

Skills: Agility and Strength are his strongest, while Stamina is his weakest.

Sword Type: Two-handed sword.

Backstory: Yui was a counseling program that helped with players feeling stress within the game, Thanks to Kayaba trapping people within the game she was removed rejected and lost her power. She roams the game without much of her memories.

Skills: Agility and Stamina are her strengths, Strength is her weakness.

Giorno is a rather unique looking kid, with his distinct gold hair and blue eyes, but he looks and acts more mature than normal teenagers.

His avatar takes that distinction and tosses it in the trash, making him look more like a normal teen.

Unique sword skill (must be unique to each character): Gold Experience (Golden Wind for localization)
Either overcharges the enemies senses to immobilize/stun them for a 5 seconds or restore life to a ally/squadmate. has a 30 seconds cooldown for each use and 30 seconds to change between modes.

Spoiler: Haruno/Giorno Backstory.

Unwanted by his mother and abused by his step father, Haruno was an easy target for bullying. However, that all changed when he spared the life of a gangster from certain death. The gangster made life better for Haruno and gave him a second chance at life. He changed his name to Giorno and now wished to protect others and be a gangstar, just like his role model. However, Giorno picked up SAO and was intrigued about how in depth it is.

Wanting to try SAO out, Giorno bought it and decided to see for himself how good this game could possibly be.

Ryuko is a young woman with medium short dark blue hair with a single red highlight. Her avatar mirrors her real life appearance.

Spoiler: Ryuko backstory

Not actually human, Ryuko was created for the sole purpose of being tested on. After seemingly failing an experiment, she was presumed dead and left for dead. She was rescued by her step father and taken in by him. despite being a sweet child at first, her attitude took a turn for the worst and she started lashing out at others, causing fights and being expelled from multiple schools in Japan. Her last chance? Honnnouji Academy, run by a tyrant called Satsuki, was the final chance she had to make amends.

Taken in by a new friend with a copy of SAO, Ryuko was intrigued by the second life part about the VRMMORPG and gave it a chance.

Aiden was a caring and thoughtful person, who became a hacker to meet up with these ideals. One day, he attempted to break into a suspicious bank account and see what was going on. However, This failed and in the end, it costed the life of his innocent niece. After this happened, he changed drastically, becoming a vigilante hoping to save as many people as possible. So naturally, when a few contacts of his detected some suspicious activity within the SAO game, Aiden was the first person to step in and check for himself.

Aiden joined SAO out of pure curiosity to see what all the hype is about, as well as to check some suspicious activity within.

Recover 25% of health
Feat4. Catapult:
Call a catapult strike over an area that deals heavy amounts of damage but only if they are in the target zone.
Feat5. Execution:
When finishing off an enemy, he can do an execution which takes longer than simply killing the opponent, but allows him to gain some health back, which depends on the amount of time he spent executing the opponent. The Execution can be interrupted, which stops him from gaining any health.
Feat6. REVENGE:
When outnumbered or outmatched in battle, if he survives for a certain amount of time in that situation (maximum of 60 seconds, which can be lowered by five seconds each time he blocks or parried an attack), he is able to activate revenge, which envelopes him in a golden glow that pushes nearby enemies to the ground, the effects are increased damage, endurance, unlimited stamina, and grants a golden ‘shield’ over his health bar that protects him from further harm, but the shield can be broken and leave him vulnerable in this state.

Throughout his life, he always had a fascination for the knights of the medieval ages and the weapons they used in combat so much that he changed his own name to that similar to a Latin one, when SAO came out, he was able to finally achieve his dream of becoming a knight, or one in a virtual world to be exact. He had been playing this game as soon as the beta came out, allowing him to get an head start in the game. But when the final day has arrived for the beta and he is now trapped in the game, he would try his best in order to help others in order to survive in this ‘Hell’ with his older brother. He also managed to ‘somehow’ get an Norwegian Lemming as a pet which may be small rodent, but is very aggressive and will try to fight things larger than itself.

The Flames of the White dragon: Once activated, his Nodachi would set aflame with a blue color and would be given temporary control over fire, controlling the direction it goes and the intensity of it. This lasts for up to two minutes in combat, and requires eight minutes in order to recharge.

Backstory: He decided to join a week before the full release only for the reason that his brother was playing it and leaving him out of it, but he would not agree to that and say something else. Inside, he would mostly travel with his younger brother or do things by himself, becoming addicted to the game but mostly only playing when his younger brother is playing. After being trapped inside the game, he will make sure that he will return to his wife and kids and make sure that nothing will hurt his younger brother and those who do will die a painful death.
