No, they can have one form and for a mantis-like monster I think it's fine if they have a humanish look as long as it has a very clear indication that it's not human.
Alright and I don't see why not. I honestly prefer having my character slowly showing their backstory to people they get close to, but a few people like having it there so it's an optional feature. :)
