My players failed to complete a quest that involved a crazy mage trying to make a machine that would open a steady hole to the fire realm, it did not work out as he planned and since the player ignore all the warnings (earthquakes) and failed to stop the events from happening the whole thing exploded and left a giant hole of molten lava in the city.
